Accessibility Grant Program

The Métis Nation – Saskatchewan (MN-S) Accessibility Grant Program is an annual income supplement to assist qualified Métis Nation – Saskatchewan families. The AGP is designed to address the needs of families whose children have unique and specialized needs.

Samples include but are not limited to:

Children with medically fragile needs
Children requiring developmental support services such as Speech and Language
Occupational Therapy
Behavioral interventions
Specialized orthopedics
Supportive technology aids
Specialized support such as adapted clothing, and unique nutrition needs.

Each child will receive up to $5000 annually to cover these unique expenses. These grants can be applied for annually to support children ages from birth to 8. Children must be registered with Métis Nation-Saskatchewan.

*MN-S will issue an annual T4A form to include in income tax planning. Families will receive the total amount they have received from MN-S for declaration on their income tax statements.

Verification of Unique and Specialized Needs:

This form is to be completed by a qualified practitioner (i.e., medical doctor) for the purpose of determining eligibility for Accessibility Grant Program.

Qualified Practitioner Information:

All the medical information must be completed by a licensed physician or other qualified practitioner.

Eligibility for funding is based on the permanence of the child's needs and the functional limitation of these needs impacting their ability to fully perform daily activities.

Please fully answer the questions below with information about your patient's functional limitations. If you require extra space, please attach a letter with the additional information.

Note that the applicant is responsible for any fees incurred to complete this form, in case of approval of Accessibility Grant, fee incurred will be re-imbursed.
